Case Manager – Consumer-Directed Services (CDS)

Job Description
This case management position supports people self-directing care in their homes and serves as an assessor, completing reassessments annually, which help determine eligibility annually for consumers receiving services in their home.

This position reports to the Director of Programs and is responsible for assisting persons with disabilities achieve goals for more independent living through the use of our consumer-directed personal care in both Veteran’s – Directed Care (VDC/VA) and Medicaid-Funded Consumer Directed Services (CDS).

Job activities include the following: eligibility determination; case management services, maintaining documentation, plans of cares, and budgets; and consumer education about how to appropriately use the program, skills training and information and referral services. The Independent Living Specialist advocates for the needs of persons with disabilities, documents services provided, makes community presentations and maintains resources to share with consumers to support their independent living.

Education and Experience:

  • bachelor’s degree from an accredited college or university; OR
  • Be a Registered Nurse (RN) who is licensed and in good standing in Missouri; OR
  • Be a Licensed Practical Nurse (LPN) who is licensed and in good standing in Missouri with one or more years of experience working as an LPN

Duties:

The responsibilities for this position include but are not limited to:

Complete intakes as they are assigned.

Monthly contacts with each consumereither by phone or in person, every 30 days

One annual face to face visit

Maintaining accurate quarterly reports of data from caseload

Educate and provide oversight of client timesheets electronically to ensure accuracy of services provided per their plan of care.

Provide case management services and advocacy including locating resources and making referrals to other agencies as needed and up to date documentation and organization of Consumers file.

Assist the Consumer in developing adequate Emergency plans, update as needed and annually.

Monitor the health, safety, and welfare of each consumer.

Completion of paperwork as required by OMO and DHSS guidelines.

Maintain assigned outreach: hanging flyers, sharing social media information, educating community members, attending Chamber of Commerce meetings, etc.

Provide Advocacy, Information and Referral, and Independent Living Skills Training.

Complete state-mandated reassessments

Job Type: Full-time

Pay: $33,280.00 – $37,440.00 per year
