Job description

The Youth Transition Specialist reports to the Director of Programs. As a Youth Advocate, the

Independent Living Specialist (ILS) is responsible for partnering with area schools and assisting youth

with disabilities through educational advocacy, high school to college transition, and other youth

transition related activities.

Essential Duties and Responsibilities:

This position is responsible for assisting persons with disabilities achieve goals toward greater

independence. This includes but is not limited to: case planning services, skills training, information

and referral services, advocacy and support.

The ILS works with consumers on individualized goals, documents services provided, participates in

community activities and presentations and conducts intakes for new Independent Living consumers.

The ILS will also assist with peer support and assist with providing resources, teach a self

determination class, assist with Disability Awareness month in the schools and participate in the

annual Youth Transition Fair.

This position is responsible for oversight of the Youth Advocacy program and creating the hard file

documentation required to open the service and the case management of the services and the

databases necessary to administer the programs.

This position will be cross-trained in TAP-T/TAP-I, Assistive Technology, Home Automation,

Emergency Funds, Emergency Management, Fundraiser Program, Equipment Loans, Circuit

Breaker, Donations and the 5 core services; Advocacy, Peer Support, Independent Living Skills

Training, Transition and Information and Referral.

Additional responsibilities and/or assignments as determined by the Director of Services for

Independent Living and/or the Center’s Executive Director.

Education:

A Bachelor’s Degree in Education, Social Work, Psychology or other related field preferred with two

years experience in Independent Living OR a combination of education and experience, with one

year of experience being equivalent to a year of education.

Knowledge, Skills and Abilities:

Excellent skills in organization, communication and personal experiences with disabilities.

Understanding of and adherence to the philosophy of independent living and the disability rights

movement.
